The FDA approved Eli Lilly’s Foundayo (orforglipron) on April 1, 2026 for adults with obesity, or adults with overweight who also have weight-related medical problems. It is a small-molecule GLP-1 pill that can be taken any time of day without food or water restrictions — unlike oral semaglutide, which must be taken on an empty stomach. In the ATTAIN-1 trial, adults who stayed on the highest dose lost an average of 12.4% of body weight (about 27 lb) vs 0.9% on placebo — 11.1% vs 2.1% counting everyone who started, per the FDA label. LillyDirect accepted prescriptions immediately and began shipping April 6. Self-pay starts at $149/month for the lowest dose; as of September 2026 Lilly lists $149-$349 depending on dose.
