Foundayo is Eli Lilly's orforglipron, launched April 2026 — the first small-molecule oral GLP-1 receptor agonist. Unlike Rybelsus it has no food or water timing restriction, though efficacy is lower than injectable GLP-1s.
Start 0.8 mg once daily, any time, with or without food. This starter dose is sub-therapeutic — mild appetite change at most, and it exists to limit nausea while your gut adapts.
Step up to 2.5 mg (after ≥30 days), then toward 5.5 mg. Appetite suppression becomes more noticeable; GI side effects (nausea, constipation) can flare for a few days after each increase.
Continue climbing in ≥30-day steps (9 → 14.5 → 17.2 mg) as tolerated. Weight loss accelerates through this stretch; the loss is stair-shaped, not linear — a faster week after each step, then a slower one.
