Wegovy retail cash-pay is $1,349.02 per month at standard pharmacies. NovoCare direct cash-pay reduces this to $499/month — same drug, manufacturer-direct, valid in all 50 states. The 63% discount is automatic when you order through Novo Nordisk's direct-pay program with a valid prescription.

If your commercial insurance covers Wegovy after prior authorization, the manufacturer savings card brings the copay as low as $25/month for up to 12 months. Standard out-of-pocket copays without the savings card range $50–$150/month depending on plan tier. Medicare Part D covers Wegovy only for cardiovascular indication; weight-loss indication remains excluded.
Three programs reduce Wegovy cost: (1) NovoCare savings card — $25/month if commercially insured with coverage, valid 12 months; (2) NovoCare direct cash-pay — $499/month if not insured or uninsured for Wegovy; (3) Patient assistance program for income-eligible uninsured patients.
Medicare Part D excludes weight-loss GLP-1 prescriptions by statute. The only Medicare Wegovy path is the cardiovascular risk reduction indication (FDA-approved March 2024) — requires documented established cardiovascular disease (prior MI, stroke, or revascularization).
Medicaid coverage varies by state. As of 2026, 16 states cover Wegovy for adult obesity through their Medicaid programs (full or restricted PA). 34 states do not cover. Check your state Medicaid formulary or our state-by-state Medicaid coverage page for verified status.
$199/month flat — same active ingredient (semaglutide), not FDA-approved as Wegovy. 503A pharmacy compounding. Regulatory wind-down ongoing post-shortage end.
$549/month via LillyDirect — different molecule, head-to-head superior weight loss in SURMOUNT-1 trial. Same FDA-approved indication for chronic weight management.
$1,300/month retail — daily injection instead of weekly. Older drug, FDA-approved for adults + pediatric 12-17.
Wegovy out-of-pocket cost ranges from $25/month (covered + savings card) to $1,349/month (retail cash, no programs). The realistic cash-pay floor for most patients is $499/month via NovoCare direct, or $199/month if you accept compounded semaglutide regulatory risk. Get coverage first, NovoCare second, compounded last.
