For the first time there is a real choice of GLP‑1 pills, not just the injection. Three are now FDA-approved — and they differ more than the label suggests, especially on the food rules that make or break how well they work. Here they are, head-to-head.

The single biggest practical difference is not efficacy — it is how you have to take them. Oral semaglutide (both oral Wegovy and Rybelsus) is a peptide with fragile absorption: you take it first thing on an empty stomach with no more than 4 oz of plain water, then wait at least 30 minutes before eating, drinking anything else, or taking other pills. Get it wrong and much of the dose never absorbs — which is the leading reason oral semaglutide underperforms in real life. Foundayo (orforglipron) is a non-peptide small molecule and has no timing rules at all — any time, with or without food. For a lot of people that convenience is the deciding factor.
In the trials, Foundayo delivered around 12% average weight loss (ATTAIN program) and oral Wegovy about 13–14% (OASIS program) — genuinely close. Rybelsus, dosed for diabetes rather than obesity, comes in lower. All three sit below the injectables: injected Wegovy averages ~15% and Zepbound up to ~21%. If you want a pill and value convenience — no morning ritual, no waiting to eat — Foundayo is the natural pick. If you want the highest oral weight loss and can commit to the empty-stomach routine, oral Wegovy edges it. Rybelsus mainly makes sense if you have type 2 diabetes.
