List prices barely moved — Wegovy is still $1,349 a month at list. The drop is in the manufacturers' own cash programs: LillyDirect began selling Zepbound vials in August 2024 and cut prices in February and December 2025; NovoCare launched Wegovy at $499 in March 2025 and Ozempic in August 2025, then cut both to $349 in November 2025. Novo has also announced a list-price cut to $675 for Wegovy and Ozempic effective January 2027.

Methodology. The index is the simple average of manufacturer self-pay prices (maintenance doses) for Wegovy, Zepbound and Ozempic in each month where all three had a manufacturer cash program, normalized to 100 at Aug 2025. Prices carry forward between announced changes and are re-checked against the manufacturers' own pages (last: 2026-09-14). Mounjaro is excluded — it has no manufacturer self-pay price with a documented start date. Cash-pay only — insured copays and compounded routes are covered on each drug’s cost guide.
